Access support process
1. Register for our services
Register using the online form or by phone on (03) 9830 2122 or 1800 777 175 or email at info@mnd.org.au
2. Appointed an MND Advisor
After registration, you will be appointed an MND Advisor.
MND Advisors are to able to provide support in accessing services via My Aged Care.
3. Make use of our services
You’re now eligible for MND Victoria Support Services. See below for some of the services available.
My Aged Care
There are three main types of My Aged Care support available for people diagnosed with MND aged 65 years or older:
- Support at Home (SaH) Program
- Commonwealth Home Support Program (CHSP)
- Residential Aged Care
You need to be assessed for eligibility before you can access these options. You can register for a face-to-face assessment through the My Aged Care website or by calling 1800 200 422. The assessor will decide on eligibility and priority. An assessor will contact you to arrange an aged care needs assessment.
MND Victoria receives grant funding under the Commonwealth Home Support Program for Individual Social Support, which contributes to our MND Advisor Program. As part of this funding contract, it is a requirement that clients accessing services under Individual Social Support and or home adjustment.
- register with My Aged Care and
- have a “referral code” for Individual Social Support, and Home adjustment
- Receive a Service Agreement from MND Victoria
- Provide consent for your data to be used for reporting purposes
- Receive a copy of the Statement of Rights and Code of Conduct
Your MND Advisor can refer, advocate, and assist you to access supports. Our services are offered at no direct cost to the client, please refer to MND Victoria’s Client Contribution Policy or speak with your MND Advisor for more information.

Services we provide
MND Advisor and Support Coordinator service
MND Advisors provide support to people living with MND to assist them to live as long as possible with the best quality of life possible.
Equipment service
MND Victoria has developed an extensive range of assistive technology equipment, available free of charge to people with MND.
Access and Education support service
The Access and Education team provides comprehensive education and information support to people impacted by MND as well as being the first point of contact for new clients and processing new registrations.
Carer programs
MND impacts more than just the person living with the disease. MND Victoria strives to provide appropriate levels of support to those who are providing care and support to their loved ones.
Support groups
MND Victoria offers a range of support group programs to support people with MND and their carers, family members and friends.
Volunteer support services
There are a range of services available including hand and foot massage, social visits/phone calls, support to use iPads, writing your life story.
