Home adjustments

MND Victoria has Commonwealth Home Support Program (CHSP) funding to help keep people living with MND safe and mobile.

A CHSP Home Adjustments request must be submitted by an appropriate Allied Health Professional.

1. Assess your client
   Assess your client and their home environment to identify a need for any CHSP Home Adjustment.
2. Obtain quote/s for modification/s
   For any works over $1500, 2 quotes will be required; for modifications under $1500, one quote will be sufficient.
3. Complete the Online Request Form
   Ensure you provide detailed clinical reasoning for the requested CHSP Home Adjustment
   Upload all quotes with the Request Form
   You will receive an automated confirmation of submission via email. If you do not receive this email, which may take up to 30 mins, please contact the Equipment Service (ES) by email or telephone. Contact details are listed below.
4. Email acknowledgment
   You will receive a second email from the ES within 3 working days to advise minor home modification status.
