Support for People Aged Over 65 Years (Diagnosed with MND)

People aged 65 years or older who are diagnosed with MND can access aged care supports through My Aged Care.

Assessment and Eligibility

Before accessing any of these services, you must complete an aged care assessment. You can register for an assessment by:

An assessor will:

  • determine your eligibility
  • decide your priority for services
  • arrange an aged care needs assessment.

Commonwealth Home Support Programme (CHSP)

The Commonwealth Home Support Programme (CHSP) provides government-subsidised support services to help older people remain living at home.

If eligible, you will:

  • receive a support plan
  • be issued service referral codes for each approved service

You can then:

  • contact service providers directly to arrange support, or
  • ask your assessor to send a referral to a provider on your behalf.

Your MND Advisor can also assist you to connect with appropriate services.

The types of help available will vary depending on your assessment and funding level, but may include:
  • Home care
  • Personal care
  • Delivered meals
  • Community access
  • Social support
  • Home adjustment funding
  • Shopping assistance
  • Respite services
  • Community bus
  • Property maintenance
  • Allied health professionals (eg. physio) and nurses
  • Individual Social Support (MND Advisor Program)

As of November 1 2025, access to the CHSP is assessed and means tested. This means you are expected to contribute to the cost of your care. Each of the service providers sets their own prices. How much you contribute depends on your income level and the type of services you need.

MND Victoria receives grant funding under the Commonwealth Home Support Program for Individual Social Support, which contributes to our MND Advisor Program and Home Adjustment funding (in specific regions).

As part of MND Victoria’s funding contract, it is a requirement that clients accessing services under Individual Social Support and or Home Adjustment:

  • register with My Aged Care
  • have a “referral code” for Individual Social Support, and Home Adjustment
  • receive a Service Agreement from MND Victoria
  • provide consent for your data to be used for reporting purposes
  • receive a copy of the Statement of Rights and Code of Conduct

Support at Home (SaH)

The Support at Home program replaced the Home Care Packages (HCP) program and the Short-Term Restorative Care program.

Support at Home provides a package of funding to help older people with MND remain living safely and independently at home.

Funding Levels

There are eight levels of Support at Home funding, based on the level of care and support required. They range from classification 1 for basic care needs to classification 8 for high care needs. The classification level is determined by an aged care assessment. Your MND Advisor can help you understand your funding level.

  • Personal care
  • Community engagement

Additional Short-Term Funding Pathways

Depending on your assessment outcome through My Aged Care, you may also be eligible for additional funding through one of three short-term pathways under the Support at Home program:

  • End-of-Life Pathway provides additional funding for in-home care services in the final 12 weeks of life. Visit the End-of-Life Pathway web page to find out more.
  • Restorative Care Pathway (replacing the Short-Term Restorative Care Program) gives access to intensive allied health and nursing care for up to 16 weeks. Visit the Restorative Care Pathway web page to find out more.
  • Assistive Technology and Home Modifications (AT-HM) Scheme provides access to funding for assistive technology and home modifications, separate from your main Support at Home package budget. Following assessment, you may be approved for:
    • A Home Modifications (HM) Tier
    • An Assistive Technology (AT) Tier
    • Both HM and AT Tiers

MND Victoria accesses funding through the Support at Home – High Tier Assistive Technology Budget for equipment supplied by our Equipment Service. If you already have equipment on loan from us before receiving funding, your MND Advisor will discuss how this may now be funded through your SaH Assistive Technology funding.

Fees and Contributions

Funding under the Support at Home program is means tested.

Depending on your income and assets, you may be required to contribute to the cost of some services.

Your income and assets will be assessed to determine:

  • Whether you need to pay a contribution
  • The rate of your contribution

To estimate what you may need to pay, you can use the fee estimator available through My Aged Care www.myagedcare.gov.au/aged-care-home-fee-estimator

To reduce financial impact on clients, MND Victoria will offer reimbursement of any client co-contribution applied when accessing our equipment service. Approved grant reimbursements are processed within 21 days.

This can be done here or by speaking with your MND Advisor.

Residential Aged Care

Sometimes people require more care, supervision, or social contact than can be provided safely at home. In these situations, residential aged care may be considered.

Residential aged care provides:

  • 24-hour care and support
  • Assistance with personal care and daily activities
  • Nursing and clinical care
  • Meals, accommodation, and social activities

Access to residential aged care is arranged through an assessment with My Aged Care.

Making the decision to move into residential aged care can feel significant. Support is available to help you and your family make an informed choice that best meets your care needs and preferences.
