How to make an equipment request
Upon submission of the form:
- You will receive an automated confirmation of submission via email. If you do not receive this email, which may take up to 30 mins, please contact the Equipment Service (ES) by email or telephone. Contact details are listed below.
- For equipment requests - You will receive a second email from the ES within 3 working days to advise equipment request status.
- For collection requests - We will contact the relevant person when the equipment collection request is received.
Wheelchair Accessible Loan Vehicle
MND Victoria has a wheelchair accessible loan vehicle available with no hire cost through the Equipment Service. The van, a Hyundai Staria, has a range of dates available from now through to next year.
It’s provided clean and with a full tank of petrol, and we ask that it’s returned the same way. The vehicle can be booked for as little as one day or up to two weeks, making it a flexible option for appointments, family visits, or short trips away.
The van can hold up to 6 additional people (including the driver).
