Home Modification Request

Please note:

1. AREAS COVERED BY CHSP (Commonwealth Home Support Program) funding are for the following regions: Victorian: Northern, Barwon-SW, Eastern Metro, Gippsland, Hume, North Western, Metro, Southern Metro
   Tasmanian: Southern, Northern, North Western
   Areas outside these regions may be covered by MND Victoria.
2. COSTS: Minor modifications or Home Safety Equipment up to $1500 AUD will be considered. Some higher cost modifications may be considered if there are no other funds available.
   For any works over $1500, 2 quotations will be required.
3. AHP: MND Victoria will accept referrals from any qualified registered Allied Health Professionals (AHP). The AHP must have assessed the person and their home environment prior to making a referral. MND Victoria will accept referrals for people registered with MND Victoria, and who are over 65 years of age (50 if ATSI/TSI). Each referral must be accompanied by a quote for works and a clear specification of the modification or modifications required.
4. WAIT TIMES: These requests are considered on a case-by-case basis, pending funding availability from each region, and are addressed within 2 weeks of receipt of the request.
